Please bring your child with you to this appointment.
All NEW STUDENTS: must provide a certified birth certificate, immunization record,
and proof of residency. If you child is an out of district transfer, please specify on the form.

Wednesday, August 13 – This is an LATE START day. Doors open at 8:55 and the tardy rings at 9:10
