Remember that regular dismissal at Porter Lakes is to be a bus rider. If your child will be dismissed another way, please let us know.

Please bring your child with you to this appointment.
All NEW STUDENTS: must provide a certified birth certificate, immunization record,
and proof of residency. If you child is an out of district transfer, please specify on the form.
